Desert View Christian Science Care Supporting the healing ministry of Christian Science
A new nationwide benevolence fund is available to assist in paying for Christian Science nursing care. Documents Required for Admission:
Social Security Card
Current photo ID: driver's license, state ID, or Passport. If these are not available then two persons unrelated to the patient must sign a notarized document attesting to the individual's identity
Advanced Health Care Directive / Power of Attorney for Health Care
Private health insurance card, if applicable
Admissions Packet: When you have spoken with our Christian Science Nursing Department, and admission has been established, click here to download, print, and then complete the forms
Bring them with you when coming to Desert View to check-in.Some basic policies regarding services from Desert View:
The patient must be actively practicing Christian Science, striving to demonstrate healing and a greater sense of dominion through complete and total reliance on Christian Science treatment.
The applicant and his or her family must understand that no diagnoses, medical examinations, physical therapies, psychological counseling, or medicines are administered at Desert View.
Any individual wishing services from Desert View must be working with and receiving daily treatment from a Christian Science practitioner who is currently listed in "The Christian Science Journal."

Application for Services: To apply for services the individual needs to be relying exclusively on Christian Science for healing, and it must be their desire to receive Christian Science nursing services.
